Karliah doesn't show up at the Ebonmere
What happens
After dropping through the well into the main chamber, Karliah may not show up, leave right away, or arrive late, and she must be present for the whole Nocturnal conversation.
PS5-safe fix
Save before dropping into the well and reload until it works. Waiting a few hours before dropping in, and not looting Anders, may help. Stand next to Karliah when Nocturnal leaves. If the quest doesn't update after Nocturnal tells you to drink, wait a few hours.
Always make a hard save before trying a fix.
